The role:

The Care Consultant role will report to Anglicare At Home’s New Business Manager, to deliver excellent customer service to all prospective clients / families / advocates, by ensuring referrals are responded to professionally and promptly, and clients are expertly assessed. With a focus to support the growth of Anglicare At Home by show casing our point of difference and enhancing opportunities for ageing in place. This is to be undertaken in line with the organisation’s Christian vision, mission, values and strategy, and quality and compliance requirements.


Key responsibilities include (not limited to):

  • Ensuring a smooth transition from sales intake to the start of care, establishing trust and credibility with prospective clients and their families
  • Delivery of client care by comprehensively assessing clients’ needs and goals, preparing initial care plans and budgets in partnership with the client and family
  • Ensuring a comprehensive understanding of Anglicare’s strategy, aged care quality standards and relevant policies and procedures
  • Ensuring that all client documentation is completed in a timely and accurate manner.

This is Permanent fulltime servicing – Multiple Locations


Criteria:

  • Diploma of Service Coordination/Community Services, and/or equivalent relevant qualification
  • Previous experience in delivering exceptional customer service
  • Previous experience in the aged care sector and with experience in undertaking comprehensive assessments in a home setting
  • Previous experience is a sales role (highly desirable)
  • Current driver’s licence
  • Demonstrated experience in the use of technology and systems.
