Anglicare is expanding their home care services in Tamworth and are looking for a highly motivated and passionate Aged Care manager who will be our Care Partner Leader working as part of the Anglicare At Home Team; with a focus to support the growth of supporting older people in their homes. This person will be passionate about home care and about show casing our point of difference and enhancing opportunities for ageing in place.


The role:

The Care Partner Leader will manage a team of dedicated care workers to achieve the best outcomes for our Seniors in the local community to stay connected and safe living at home.

Reporting to the Regional Manager Northern Inland, the Care Partner Leader will play a key role to deliver an excellent customer experience to prospective & existing clients / families / advocates, by ensuring clients are expertly assessed and effectively case managed, in line with the organisation’s strategy, quality and compliance requirements, Vision, Mission and Values.

Key responsibilities include:

  • Ensuring a smooth transition from sales intake to the start of care, establishing trust and credibility with prospective clients and their families
  • Professional delivery of case management by assessing, care planning and monitoring client’s needs and goals, using a consumer directed care approach
  • Leading and mentoring a team of care workers
  • Contributes to the daily management of all scheduled services to ensure they are covered by the team and staff absences are managed
  • Contributes to the growth of Anglicare At Home, our Aged Care service in Tamworth and this includes developing and maintaining good working relationships with key stakeholders
  • An understanding of the Aged Care Quality Standards



  • Diploma of Service Coordination/Community Services, and/or equivalent relevant qualification.
  • Desirable, Enrolled Nurse Qualification or Aged Care Management experience
  • Previous Management experience gained in a busy environment.
  • Previous experience in delivering exceptional customer service.
  • Desirable, previous experience in a sales role.
  • Previous experience in the aged care sector, with experience in undertaking comprehensive assessments in a home setting
  • Demonstrated experience to achieve outcomes according to timeframes, deadlines and project plans.
  • Proven ability to maintain relationships with internal and external stakeholders
  • Demonstrated experience in the use of technology and systems
  • Sound written and oral communication, interpersonal and negotiation skills.
  • Fully vaccinated for Covid-19
  • Current driver’s licence, with a reliable vehicle and comprehensive insurance.
  • Must be willing to travel.
