Child and Family Practitioner - Families Together

Hope Connect – Telopea / Penrith/St Marys

Hope Connect is seeking Child and Family Practitioners for a new Families Together team under the Family Preservation reform

  • Recruiting Multiple Positions–
    • Permanent Part-time – 4 days per week (30 hours per week)
    • Permanent Full Time – 5 days per week (37.5 hrs per week)
  • Rewarding opportunity to help establish the new Families Together team within an established Child and Family Support Service.
  • SCHADS award level 4.4
  • Based in Telopea – covering the Penrith / St Marys area

About the role

Families Together Child & Family Practitioners deliver intensive, home-based casework to families experiencing complex challenges.

You will provide trauma-informed, child-centered, and culturally responsive support that strengthens families and improves outcomes for children.

You will work closely with families through home visitation and direct engagement, supporting case planning and goal setting, and providing advocacy, service co-ordination, and practical and therapeutic interventions.

Hope Connect is a not-for-profit community organisation (a community work of Epping Church of Christ, motivated by the love and compassion of Jesus for all people).

For over 50 years Hope Connect has been providing support to the local and surrounding communities and those who may be experiencing vulnerabilities.

The organisation provides a variety of community welfare services to people in the communities of Telopea, and surrounding suburbs. The range of services includes DCJ funded Family Support Service, the No Interest Loan Scheme, Material Aid, Community FoodCare, Chaplaincy, and a Community Shed.

QUALIFICATIONS & SKILLS REQUIRED

Child and Family Practitioners will be required to demonstrate the following experience and qualifications:

  • Tertiary qualification in Social Work, Psychology, Community Services or related field.
  • Demonstrated experience with providing intensive case management to vulnerable families experiencing trauma, domestic violence, child protection risk, mental health concerns, and complex psychosocial needs.
  • Demonstrated ability to assess and manage risk, child safety concerns and meet statutory obligations
  • Competent with maintaining timely and accurate case notes, records, and data collection requirements.
  • Able to work collaboratively with DCJ and relevant external service providers to support coordinated service responses and positive outcomes for children and families.
  • Provide trauma informed, culturally responsive and child-centered support
  • Strong communication, organisational and interpersonal skills.
  • Participate in team meetings, supervision, and professional development opportunities.
  • Ability to work both independently and collaboratively within a team.
  • Current Criminal History Clearance and Working with Children Check.
  • Current driver’s License, access to a vehicle with comprehensive insurance & willingness to travel.
